The only requirement for this dungeon is a heavily forested area, meaning it can be dropped into just about any world. It ties in heavily with Fey creatures so I suggest a hook regarded some ancient relic hidden away by the Fey that must now be retrieved. Perhaps the former ruler had an agreement with the ones who hid this relic away, but a new ruler has taken the seat of the Emerald Queen and all agreements have been terminated.

Taking for himself the name Mogens (“powerful”), this evil spirit, now a self-styled dark lord, gathered wicked cronies and servants to itself and came to the Valley of the Sleeping Jarls. There, he has spent nearly two decades corrupting and desecrating the sacred resting place, reshaping the hollowed tombs into a blasphemous temple to his infernal lord.

The mysteries and legends surrounding Whispering Widow Woods have always been enough to scare off the timid. The dense canopy shrouds the forest floor in darkness even at mid-day, and the tangled underbrush inhibits travel and can disorient even seasoned woodsmen. Lately, though, terrorized residents have described attacks by normally docile creatures, such as black bears. Respected citizens told wild tales of unprovoked attacks by treants, sprites, and brownies. The last two curious souls to venture into Whispering Widow Woods have not returned. Tales of Enchantment has no mechanism built- in to keep the players on track. That is part of the problem for them to solve. Vault of the Blood Mage is a deadly one-shot dungeon adventure for 4-6 8th level characters. Player Characters find themselves locked in a dungeon room with no memory of how they got there. Inflicted with a magical disease that is slowly turning them into mindless zombies, the dungeoneers have to work quickly to find the antidote and escape the vault before succumbing to the deadly affliction. To do so, the characters must avoid deadly traps, fight unique monsters, and solve fiendish puzzles.
Kingdom of the Blind is a short adventure for four 8th-level characters. The adventure is set in a minor duchy that is fairly removed from the ruler of the land. As a result, trouble can brew in the land and the king would not know immediately. The PCs faced all manner of fungus last time. Perhaps with a little more investigation, they'll discover what's really going on with Zhanna and Dephyl.
In this scenario, the adventurers pass through magical portals into a series of interlinked chambers. The characters are encouraged by a young scholarly mage to voyage into the Sea of Pastures, to explore a mysterious island connected with a number of recent shipwrecks and disappearances. The island is grassy and windblasted, but eventually the characters discover a stone door leading into a subterranean complex. There, they discover 18 rooms linked by secret passages and magical portals. Most of these rooms have been ransacked by a variety of other survivors, human and monstrous. These survivors are likewise trapped within the labyrinth and are either eking out a miserable existence there or else desperately searching for a means of escape. Also within the building are a number of extraplanar creatures, collectively known as gingwatzim, who can shift between various forms: an energy form (glowing ball of light), an inanimate form (usually a magical weapon), and an animate form (an animal or monster). Eventually the characters may find the exit, and are once again deposited on the dreary islands to await rescue. TSR 9110

Genius rogue artificer Ichabod Russet hired ten of his most brilliant colleagues to collaborate and build the greatest automaton yet invented: a clockwork beholder. He dreamed their invention would catapult him into stardom and wealth. In a secret mountain laboratory under Ichabod's direction, each scientist devoted themself to developing one ray for the creature's mechanical eyes -- but when the mysterious tenth ray was added to the automaton, it became too powerful. The clockwork beholder broke free during QA testing and destroyed half the lab. Only a few of the artificers escaped to tell the tale, and for the most part, they're keeping their mouths shut.
