In this prelude to the full Princes of the Apocalypse campaign book, adventurers face off against the influence of Elemental Cultists in the hills and canyons of the Dessarin Valley. The four mini-adventures within are only tangentially related to each-other, and instead serve as starting points and hooks for the plots of the complete Princes of the Apocalypse Campaign. Players start in the well-described town of Red Larch; first investigating a local necromancer, then uncovering cult influences within the town, and finally heading off to infiltrate or do battle with two small elemental cultist strongholds. All parts of this adventure can serve as great places to seed hooks for other campaigns.
The PCs are sent to investigate a sculpture of the goddess Ezra with seemingly miracle-granting powers. The investigation takes the heroes on a pilgrimage to a forgotten chapel, where they encounter a terrible plague, a wererat police force, death, resurrection, mist horrors, and the miracles of the statue itself.
This adventure is written with the intent that it takes place in the subterranean world called the Underdark in the world of Forgotten Realms, although it may be placed in other settings. Within the Underdark, Cyrog's lair can be placed anywhere though it is stated to be thousands of miles east of Gravenhollow. If you are running Out of the Abyss, you may want to have your players see the vision Cyrog Lives! Hail Orcus! (OOTA, 158). Additionally, it is recommended that this adventure be used for a party of 4 to 6 players, level 9 to 12. While merely hinted at through visions in Out of the Abyss, Orcus is roaming the Underdark unchallenged with an army of mind flayers at his disposal. Cyrog, the elder brain of a mind flayer colony in the Underdark has died. The demon lord Orcus has reanimated and taken control of it. The hive of mind flayers is now bent to Orcus's will through Cyrog as he strives to make all life in the multiverse into a vast undead horde under his control. The adventurers are contacted by a patron who gives them a special item (such as brain worms) to suppress their brain waves and make it possible to infiltrate the Cyrog's Lair without immediate detection. The adventurers must defeat or otherwise subdue mind flayers and a few guards, override the elder brain's mechanical defense system, and slay Cyrog.

Ten days ride north of the great city, there is a small town named Treefall. Up until recently, this town was well known for its rich harvests, happy villagers, and prosperous weather. These days, however, the once-proud details from Treefall have turned darker. Instead of tales of unending prosperity, you hear hushed whispers of a great curse that has struck the once-happy village. Word has it that, where once roamed contented cattle, now strange beasts hunt by the moon's cold light and terrorize the hapless region. You have scoffed with the others at these stories. Despite the things you've seen in you adventures with your friends, the thought that such ill might befall such a sleepy town strikes you as terribly unlikely. There is plenty of evil in the world, true, but there are also much bigger and better targets. But, tonight despite the firmness of your disbelief, the stories come to find you. You have been traveling north along the roads on your own business, enjoying the rich fall colors and warm afternoons. Your camp os small and secure, with a roaring fire and meat cooking over the flame. It's one of those crisp autumn days that makes you long for a warm hearth after a long ride in the cool air.
